Digital Authorization Form – PACS EDEN

This document serves as authorization for Nataly Imaging Services LLC to share and deliver a patient’s ultrasound digital images through the secure PACS EDEN system.

By providing authorization, the patient acknowledges that the images will be transmitted in a digital and encrypted format to maintain confidentiality. This authorization permits the release of the imaging results directly to the patient for personal reference or for sharing with their healthcare provider.

The digital delivery will be made using the patient’s contact information provided to Nataly Imaging Services.
